Background and aim: Osteoarthritis is a common joint disease in humans that can lead to disability. Its main symptoms are pain and joint inflammation. Research has shown that maintaining spiritual health can help manage the disease and relieve pain. The present study was conducted to investigate the effect of spiritual health on pain management in patients with osteoarthritis in Abadan and Khorramshahr, southwestern Iran. Materials and methods: The present study is a qualitative content analysis of 15 patients with osteoarthritis who were referred to the clinics of Ayatollah Taleghani Teaching Hospital and Shahid Beheshti Teaching Hospital in Abadan, and Valiasr Teaching Hospital in Khorramshahr, southwestern Iran, in 2023. The participants were selected through purposeful sampling. The sample size was determined based on the criteria of information saturation. The study utilized semi-structured interviews as its research method. The data were analyzed using a systematic classification and coding process based on the Graneheim method. Findings: Fifteen patients with osteoarthritis were studied, 11 of whom were female and 4 were male. Their mean age was 47±4.5 years. Data analysis yielded 68 codes. The obtained codes were categorized into 15 subcategories, which were subsequently merged into three categories: faith and trust in God (comprising 8 subcategories and 38 codes), divine providence (comprising 4 subcategories and 13 codes), and the effect of prayer (comprising 3 subcategories and 17 codes). Conclusion: The results of this study showed that a close and strong relationship with God relieves physical and mental pains, increases tolerance and adaptability to problems, hope, acceptance of divine providence, peace, and satisfaction, positive energy, increased motivation, reduction of stress and anxiety, and reduction of nosophobia. As a result, spiritual health is effective in pain management in osteoarthritis patients. The current research can be the basis for paying more attention to therapeutic interventions with a spiritual health approach for patients.
